If these or any other
volunteer opportunities are of interest to you, please download
and print the forms provided below, then either drop them off at
our office, or mail them to us. Please note that a background
check is a prerequisite to becoming a Make-A-Wish®
volunteer. In order to complete the check, which is conducted by
the Rhode Island Attorney General's Office, you are required to
provide us with a photocopy of your driver's license when
you send us the forms. A notary public is available in our
office, should you need one. (Forms are in Adobe PDF format. If
